Frozen Death Benefit

Sickness Death Benefit of 1986 Medical--Free 24 months if employee was Active and free Medical for 12 months if employee was retired (after the period that is paid for by Verizon ends, the surviving spouse can pick up the same coverage for the rest of her or his life at the group rate that Verizon would pay.

Dental--Last day of the month in which the employee dies spouse can pick up Dental for 36 months under COBRA

Vision--Last day of the month in which the employee dies (spouse can pick up Dental for 36 months under COBRA

Pension--Can be taken in lump sum or annuity if employee was active. For the spouse of a retiree it depends on the Surviving Spouse Benefit Option for the annuity that the retiree had.

Social Security Supplement--These payments stop with the death of the retiree.

E-IPP or Regular IPP payments--the Balance that is owed to the surviving spouse is paid in a lump sum.

Concession Telephone service continues for 3 months from date of death.

401K Savings Plan--The surviving spouse may receive payment as soon as administratively possible or defer payment up to the date that the employee or retiree would have reached age 70 1/2 years old. The surviving spouse has the same choice of payment options.
